Try this:

when HTTP_REQUEST {
    if { [string tolower [HTTP::uri]] starts_with "/en/" } {
        set uri [string map -nocase {"/en/" "/en-us/"} [HTTP::uri]]
        HTTP::redirect "http://[HTTP::host]${uri}"
    }
}
