APM: Logon Page (AutoLogon) does not take input ID and password

I am working on to seup APM policy to connect to VMware horizon on new BIG-IP.

This is sample to simplify the trouble. I input ID and password on the logon page, but my input ID and password is not set on session variables. Therefore, any action after Logon page fails.

Could you give me any suggestion to look into?

Any reason why you are trying to set this up manually? I would check out the F5 iApp template for this: https://my.f5.com/manage/s/article/K15041. Usually a good start for the configuration aspects for this particular application.
