 Until I can look into this further (I am not at PC right now, it is midnight in Seattle) please try this: for a test, remove Machine Info Item from your Access Policy. It is only used to get MAC addresses from clients, and if it is missing then APM-DHCP solution cannot possibly send those MAC’s to DHCP server.
