Upgrade from 11.5.4 to 13.1.0.4

We are ready to upgrade from 11.5.4 hf2 to 13.1.0.4, but the jump is so high that I want to know if someone already done it and what are the gotchas you had encountered? What process did you follow? ...

We are working with the vendor to fix this specific issue, my bet is 13.1.x still not stable version compare to 12.1.x
