I have added two LTM's and monitored them sucessfully, but we now have upgraded these to 9.4.7.320.1 however as part of this work we have also changed the management IP of these.

It's good that at least you could reinstall without having to go through a significant haul of reconfiguring the management pack. We'll look into adding support for such and similar discovery / removal scenarios in one of our upcoming releases. In the meantime we'll try to come up with a reasonable workaround for this issue.
