LTM VE 11.2 keeps going offline

The Error I am getting is for the connection to MySQL. The LTM Virtual Edition is installed on the VMWare platform. For the past couple weeks, I have been rebooting it to resolve the "OFFLINE" issue. I need a more permanent resolution.
