failed Google Authenticator Key Generator and back to logon page

hi expert

i have issue 

fisrt attemp i am not able to generate barcode

Google Authenticator Key Generator and fallback to logon page but second tried i   can generate barcode enroll with using different otp 

how to troubleshot  this issue ?  i am using debug but not found the root case
