It can be as simple as:

 

1. Create a pool with pool members (you can create members when you add them to the pool) and use an appropriate service specific health monitor (the LTM comes with a number of popular prebuilt monitors)
2. Create a virtual server and assign a VIP then attach the pool created in step one.
