Drop Down on Logon Page is braking portal access. Any ideas on how to fix this?

I'm running version 11.4.1 and using APM to create remote access portal for users. I wanted to add a drop down on the logon page and populate it with users groups so I followed the following article:

After inserting the javascript into my logon page the portal apps no longer work. They simply come back with page cannot be found. I'm hoping someone can help out here. Are there any other ways to create drop downs? or places to look at logs to figure out what happens when I try to click the portal link as nothing shows up in /var/log/apm at this point?
