I don't think you will find a full-featured app in a free codeshare repository. If you have specific business requirements for a LTM/GTM-aware change-management tool which integrates with your databases then you will either need to develop such tool yourself (if you have relevant skills and expertise) or hire a consultancy company, which can develop such application to your bespoke requirements.
