The white paper for BIP version 10.x Appendix A (Manual Configuration) has all the pertinent information needed to configure for version 9.3.1.

 

The monitor for port 135 is still something I need to work out as RPC is used for more than Exchange on the servers but configuring three virtual servers with three pools and the necessary adjustments for persistence and such works out fine.
