ASM pre-check list

Good day,

 

I would like to provide our clients with a document that has a table/list of information to populate before an ASM install takes place.

 

As an example the client should provide us with

 

VS PORT BACKEND BACKEND PORT SERVER TECH APPLICATION LANGUAGE

 

Does such a template already exist or is there a link that provides guidance on what is required from the customer/application owner/developer?
