APM login page customization

Hello,

 

Currently running v11.3 LTM/APM mods and trying to customize the look and feel of a log on page for a access profile. Very simple profile configured to authenticate the user to a vip. I'm using the advanced customization option and trying to align what I believe are the fields where the user enters their username and password with the labels to the center of the screen. It seems like everything I try to adjust does not bring them over more to the left. I believe I need to keep editing the Form Settings and Form Elements, but I'm not 100% sure...

 

Anyone have an idea for how I can do this? Here's the advanced config pasted below and I'll try and include a screenshot of the page so you can see the issue and what I'm trying to describe. In the screenshot you can see the field boxes way over to the right and I need to bring them over next to the username and password labels. The form needs to stay the width of the page though.
