Try this

when HTTP_REQUEST {
  if { ([IP::client_addr] eq "<sourec ip1>") || ([IP::client_addr] eq "<sourec ip2>") } {
    if { !(([HTTP::host] eq "<x.x.x.x>") || ([HTTP::host] eq "<y.y.y.y>")) } {
      reject
    }
  }
