It looks like you need to go to:

 

https://devcentral.f5.com/s/articles/mysql-proxy-iapp

 

Which is where you would go if you clicked on 'iApp' in the sentence "This proxy is also available as an iApp."

 

And download the zip file. Which looks like an iApp template to create everything for you and the iRule code is in it.
