WEB Application Firewall Installation Questionnaire

Hello   Is there any questionnaire for WAF Installation? I need to give that to my customer to fill and after that optimize and confiugure the WAF?   Thank you

Is there any Document or questionnaire that can i download and use that? How about the other parameters like http/https connection per sec and etc. i need a sample questionnaire form to configure the ASM.
