Pool member status page on v11

I'm running 11.4.1 and have - for the most, successfully configured a pool member status page (example https://devcentral.f5.com/wiki/iRules.Pool__Member__Status__Page_on_a__Virtual__Server_v10.ashx?NoRedirect=1). The only issue I am having is when my pool members are named vs. just the IP, it's not working.
