LTM Transparent proxy deployment challenge

We want to deploy four LTM instances with 4000 pool members running on each instance. Two application servers with 8000 TCP Port are behind LTM. LTM needs to monitor each individual TCP Port of application server and based on that fail-over the service to other server. Scenario is as below.

 

 

Customer has considered four different LTM instances to manage different port range for better operation perspective. we see one challenge with above scenario is application sever wants to capture client IP address and can not configure X-forwarded-for since it would TCP Virtual server. hence only option is to change server gateway to LTM IP address but we can not keep Four different gateway on application server since we would be using four different instances for different Port range and application servers are same.

 

Please provide suggestion/solution to achieve above requirement.
