How can i get LDAP format from AD to add it on AD Query, due to i need to restrict the access by AD Group ( if User is a member of specific AD Group)

i have added AD Query item and all parameters,but unfortunately it is not working find. the error is "Your session could not be established.
The session reference number: XXXX
Access was denied by the access policy. This may be due to a failure to meet access policy requirements."
kindly note that the our goup is examble.company.com/x/y/z/NameOfGroup and it has been added as below parameter CN=NameOfGroup,OU=x,OU=y,OU=z,DC=examble,DC=company,DC=com
let me know if is there any missed configuration (See below image).
