I know ISA has a limit on the number of connections to a specific IP. I ran into this before load balancing connections to an ISA box. Not sure if the number of connections per IP is an option for ISA. In my case, I was SNAT-ing the traffic so it all appeared to come from the same IP (the egress of the LTM).
