We are in need of creating a vip https://some.site.net:6749, offload https, accept the additional port and send to the host on port 6749.

A VIP will support only one port.
When the user sends a request https://some.site.net:6749, the LTM will look for a vip created for port 6749. Hence, you can create a VIP on port 6749 and configure the SSL client profile to offload the SSL. Create a pool with the destination host with port 6749 as member and add it to the VS.
