F5 not distributing the request

As a surprise F5 loading request to one instance until it mark as it died and then takes the next instance and bump with highest load for the next instances in round-robin fashion, the actual implementation of LB was not using iRule rather it was customized with cookies+jvmid session management. What might be the cause for odd behavior of F5?.
