If you have APM policy for Exchange may I ask you to check EWS health on Microsoft Remote Connectivity Analyser?

We use Forms based (client-initiated) for OWA
Other services are disabled

Can you assign a log profile with debug enabled on SSO?
Moreover have you checked that your exchange installation supports NTLM?
