An item that generates a 404 error won't necessarily be consdiered a bad request or a response so the headers will likely not be dumped to the pvac log. To see what content is generating a 404 or 503 you can enable HTTP logging in the policy. The HTTP logging along with the logging enhancement in 9.4.5 HF 2 should provide you with all the details you need on where the errors are coming from.
