Hi,
Can I directly upgrade from 11.5.2 to 12.1.2 on LTM 4000s?

But wanted to confirmed with you guys :)
I will appreciate if you can help!

Hi!
I tried something similar a few weeks ago and it worked just fine. Activate the license before doing it. Also, try the iHealth upgrade advisor to catch known caveats between the two.
