PDF Configuration Form

I've seen somewhere on either here (Devcentral) or the main F5 site a nice PDF form for documenting/capturing all the configuration for a Virtual Server (VS/Pool member IPs, Monitors, etc). I was trying to point a client towards it, but can I find a link for it....!?

Does anyone know what I'm talking about and can direct me please, if it still exists out there somewhere?
