LTM Cisco ACS 4.0 RADIUS authentcation

I have setup my ACS RADIUS and LTM, also configured static route on the LTM so that all RADIUS tx will pass through the management port.

 

 

However, I can't seem to make it work. In Cisco ACS, I always get an error "CS Password invalid", but I'm quite sure I got the password right.

    It looks like I pass authentication, but get "Users Access Filtered" and "No Access Filters Passed." in the ACS Logs. This is with Cisco SecureACS 4.1
