LTM Caching

I have had some of my devs complain that when they make a change to a site behind the LTM, the changes are not visible on the site.

If we turn of caching on the LTM then the changes on the web site are visible.

So my question is, how does the LTM know that something on a website has changed, and to refresh it cache?
