Yes I think, You can try with your LTM Policy.
In your LTM policy, When your condition Match then your action is to FW to specific pool.
Add Action before your action FW to pool:
- Remove http header "Host" Request
- Insert http header "Wanted hostname" Request
- FW to pool
You can also try replace for http header.
