Old topic but not much information available. 

We successfully used iRule LX, node JS and a per-request policy for MFA authentication with the Radius Fortiauthenticator API. 

PUSH OTP SMS EMAIL, could be configured if desired.

Our solution has been shared with both F5 and Fortinet support.
