Exchange 2013 advanced monitor for OWA

Using LTM ver 11.2 HF4 and we are configuring Exchange 2013. By using the iapp template (v1.2.0) it asked if I wanted to use advanced monitoring. We would like to use more advanced monitoring, however the option for OWA using forms-based auth and requiring the CAS servers to only use a username instead of domain\user will not work for us. Does anyone have an example of a custom monitor that will use these advanced monitors but by using domain\user for auth?

 

I'm going to try and install this in my lab to see what the advanced monitor looks like but thought I would ask.
