DUO/Radius Auth - APM Customization

I customized my APM Stylesheets about 6 months ago to match a better looking portal for my organization.

Now I am attempting to use DUO and integrate it into my F5 APM Portal Page. The problem that I am having is with a customized Style Sheet while using DUO, I get a Labl Box showing - "Duo Initialization" and a Text Box requesting input, just above where the DUO push,send, and sms notification box appear.

I tested with a Default APM Stylesheet and I do not get the same response.

Does anyone know where, within the "Radius Auth" APM policy a Label and Text Box would be generated from? Or where I would go to Edit and remove the Label and Text Box within the Style Sheet that is popping up at the DUO Screen.

I followed the directions here, https://duo.com/docs/f5bigip and within the F5 Lab Docs out on Dev Central. It works with a non modified style sheet but does not work with a Modified Style sheet.
