Is there a field to enter the NAT address for the BIG IP APM? Are we supposed to have the APM behind our firewall?
Is your virtual server using source persistence? If not, use source persistence and see if the problem continues.

After changing the persistence profile to the source address the connections are being established correctly. I will keep the changes to see if there are any issues, but that appears to be the correct setting.
Lesson learned, don't use "F5's recommended persistence profile"!!!!
Actually, I believe you uncovered a error within the iApp configuration. It should be adding source persistence instead of using universal persistence for your specific use case were APM is not being used (the universal persistence profile created and applied does not have any effect since it persists on APM session cookie). I have put in a request to have it fixed, thanks!
